Learn more about the Outcomes Congress Partnerships for Health (PFH) is an innovative South West LHIN initiative that helps primary care practices improve their diabetes care through an inter-disciplinary, team-based approach. Already more than 50 practices are involved. Many are demonstrating positive results, with patients showing improved A1Cs, blood pressures and blood lipids, and fewer low blood sugar incidents.
