What is the Outcomes Congress?
It is the public forum at the end of the collaborative where the experiences of the participants are presented to others. In this case, more than 70 primary care practices, CCAC providers, DECs and other community partners have been engaged in a three year process to transform the way care is provided to patients with diabetes in the South West LHIN. The Partnerships for Health (PFH) Outcomes Congress will highlight the data and successes to show that there is a way to change community and primary care for a marked improvement in chronic disease prevention and management.

Why should I attend?
The Outcomes Congress is an opportunity for participating teams to share their care transformation experiences. It is an opportunity for the audience to learn processes and techniques from them. The intent is that by sharing the information, this will help spread their improvements to other practices, to other organizations, and to policy makers.

As well, PFH is pleased to note that this program meets the accreditation criteria of The College of Family Physicians of Canada and has been accredited by the Ontario Chapter for up to 4.5 Mainpro-M1 credits.
