Your team will:

  • Receive education on the latest research on diabetes management
  • Learn quality improvement techniques that improve patient outcomes
  • Receive professional practice coaching to implement quality improvement efforts
  • Learn how to get meaningful data from your EMR
  • Learn how to empower patients to care for their own health
  • Form partnerships with other community agencies to achieve improved collaboration in care for your patients

You choose how you want to be involved:

  • Through team-based action learning (involving 2.5 days off-site), supported by coaching
  • Through lectures (involving one day off-site) and monthly teleconferences, supported by coaching
  • Through self-directed web-based learning, supported by coaching
  • With coaching only

And remember:

  • You will receive a monthly stipend
  • This program has been reviewed by The College of Family Physicians of Canada and is awaiting final accreditation by the College’s Ontario Chapter.
