Diabetes: Clinical Plenary - Dr. Stewart Harris
Dr. Stewart Harris holds the McWhinney Chair for Studies in Family Medicine at the University of Western Ontario. His research focuses on Type 2 diabetes in high-risk populations and improving the management of diabetes in primary care. He is a member of several national diabetes organizations, and serves as Chair of the Clinical Practice Guideline Committee of the Canadian Diabetes Association.
CCAC Potential - Sandra Coleman
Sandra Coleman is Executive Director of the South West CCAC. A former lawyer, she is a member of the Board of Directors of the Ontario Hospital Association, and the Thames Valley Family Health Team.
Integration - Eileen Patterson
Eileen Patterson is Director of Quality Improvement for the Ontario Health Quality Council. Among many other contributions, Eileen led the groundbreaking Taber Integrated Primary Care Initiative and Alberta's Chronic Care Self-Management Initiative.

Mike Hindmarsh is President of Hindsight Healthcare Strategies. He is the former Associate Director of Clinical Improvement at the MacColl Institute for Healthcare Innovation, where he and his colleagues created the highly-regarded Chronic Care Model.
