Project Participants
Partnerships for Health provides a unique opportunity to apply the Ontario CDPM framework to practice. The initiative integrates the health system by sharing information across the continuum of care; advancing partnerships amongst primary care, CCAC, diabetes education programs, mental health and other community support services; strengthening linkages to tertiary care; engaging the patient in self-care; and enabling improved information management. Ultimately the goal is to improve care to individuals with diabetes and equip participants with the knowledge and skills to apply the approach to other chronic conditions. South West Community Care Access Centre
Coordinates community-based care and support and long-term care admission. The South West CCAC also serves as the transfer payment agency for the intiative providing oversight of the project agreement and funding arrangement with the Ministry of Finance.

South West Local Health Integration Network
A crown agency responsible for planning, integrating and funding more than 150 health service providers including hospitals, long-term care homes, mental health agencies, community support services and the South West CCAC, in the South West region. Partnerships for Health is sponsered by the South West LHIN.

 

Thames Valley Family Practice Research Unit, The University of Western Ontario
TVFPRU is conducting an independent evaluation of the initiative.

Other partners include community and teaching hospitals, Diabetes Education Centres, pharmacists and specialists.
