Partnerships for Health is an innovative initiative in the South West LHIN that helps primary care practices improve their diabetes care through an inter-disciplinary, team-based approach. Already over 50 practices are involved. Many are demonstrating positive results, with patients showing improved A1Cs, blood pressures and blood lipids, and fewer low blood sugar incidents.
