Partnerships for Health (PFH) was an innovative South West LHIN initiative that engaged health care teams to transform the way they provide care to those with chronic disease.

Although PFH formally concluded on March 31, 2011, work from this successful quality improvement initiative will continue with the Quality and Process Improvement Program (QPI) as Partnering for Quality in Chronic Disease Care.

The focus of 2011-2012 is on sustaining the gains from Partnerships for Health. Visit the QPI Program website, Quest for Quality, to learn more about the history of Partnerships for Health and to learn how the QPI Program is supporting quality improvement in chronic disease care across the South West.
